Make up Class Request Form
Fill-up the form below to request for a make up class.
Make up Class Request Policy: To be eligible for a make up class, you must report an absence before the start of the class. Make up classes are assigned on a first come first serve basis and are dependent on availability. Priority will be given to students who have are requesting their first make up class. It may not be possible to accommodate players who make multiple requests per month. If a student is registered to play 1 time per week, they may request 1 make up per month. If a student is registered to play 2 times per week, they may request 2 make ups per month. If a student is registered to play 3 times per week, they may request 3 make ups per month.
